§1 General provisions

These Terms & Conditions govern the use of the online shop and stationary services available at sibowcentrum.pl, operated by Sibo W Centrum, ul. Nowogrodzka 18A/3, 00-511 Warsaw, Poland, e-mail: kontakt@sibowcentrum.pl (hereinafter: “Seller”).

Using the shop or services constitutes acceptance of these Terms. These Terms are available free of charge at sibowcentrum.pl/en/terms.

§2 Products and services

The Seller offers:

Breath test home kits (sent by post) — for self-conducted testing for fructose, lactose, sorbitol intolerance and small intestinal bacterial overgrowth (SIBO/IMO). Each kit includes instructions, sample collection materials, and transport components.
Stationary tests — breath tests performed at the Seller’s clinic in Warsaw.
Dietitian and medical consultations — available in-clinic or remotely, by prior appointment.

All prices shown on the website are gross prices (VAT included) expressed in Polish zloty (PLN). The Seller reserves the right to change prices and organise or cancel promotional offers.

§3 Placing and processing orders

Orders may be placed 24/7 via the order form on the website. An order is accepted for processing upon receipt of an e-mail confirmation. The Seller reserves the right to refuse an order if the product is unavailable or if the Customer has provided false information.

Stationary appointments and consultations require prior booking via the booking form on the website or by phone.

§4 Payment

Available payment methods: online bank transfer, payment card — processed by certified payment providers (Przelewy24, PayU, or others). Available methods are displayed during checkout. Orders are dispatched after payment is confirmed.

§5 Delivery of kits

Breath test kits are shipped within Poland and to selected EU countries by courier or postal service. Estimated delivery time is 2–4 business days from dispatch. Shipping costs are shown in the order summary before checkout.

After completing the home test, the Customer returns the samples to the laboratory address provided in the kit instructions. Return shipping costs for samples are borne by the Customer unless stated otherwise in the product description.

§6 Complaints

Complaints regarding products or services should be submitted to kontakt@sibowcentrum.pl, including a description of the issue, the order number, and proof of purchase. Complaints are handled within 14 business days. Detailed warranty rules are governed by the provisions of the Polish Civil Code.

Returns and complaints address

All parcels related to a product return or complaint should be sent to the following address:

Before sending a parcel, the Customer should contact the Administrator/Seller at kontakt@sibowcentrum.pl to arrange further steps.

Return or complaint of a mail-in test

In the event of a return or complaint regarding a mail-in test, the Customer will, after prior contact with the Administrator/Seller, receive information on the procedure to follow and on any possible shipment of the test or sample.

Parcels should be sent to the registered office address: SIBO w Centrum Diagnostyka Sp. z o.o., ul. Nowogrodzka 18A/3, 00-511 Warsaw, Poland.

§7 Withdrawal and returns

The rules governing withdrawal from the contract and returns are set out in the separate Returns Policy available on sibowcentrum.pl. Due to the hygienic nature of the products, test kits are non-returnable once the packaging has been opened.

Payment refunds

Where a complaint is accepted and a refund is granted to the Customer, the refund shall be made no later than 14 days from the date the complaint is upheld.

As a rule, refunds are made using the same payment method that the Customer used when concluding the contract, unless the Customer accepts a different form of refund. The Customer is not required to provide a bank account number. If a refund to the original payment method is not possible, the refund method will be agreed individually with the Customer.

§8 Personal data

The processing of personal data is described in the Privacy Policy available on sibowcentrum.pl. The data controller is Sibo W Centrum.

§9 Final provisions

Matters not covered by these Terms are governed by Polish law, in particular the Civil Code, the Consumer Rights Act, and the Act on the provision of electronic services. Disputes shall be resolved by the court with jurisdiction over the Seller’s registered address, subject to consumer protection provisions on court jurisdiction.

The Seller reserves the right to amend these Terms. Changes take effect no earlier than 7 days after the updated version is published.

These Terms are effective from 1 May 2025.
